scsi: Use device_show_string() helper for sysfs attributes

Deduplicate sysfs ->show() callbacks which expose a string at a static
memory location. Use the newly introduced device_show_string() helper
in the driver core instead by declaring those sysfs attributes with
DEVICE_STRING_ATTR_RO().

No functional change intended.

scsi: smartpqi: Replace deprecated strncpy() with strscpy()

buffer->driver_version is sized 32:

... the source string "Linux " + DRIVER_VERISON is sized at 16. There's
really no bug in the existing code since the buffers are sized
appropriately with great care taken to manually NUL-terminate the
destination buffer. Nonetheless, let's make the swap over to strscpy()
for robustness' (and readability's) sake.

scsi: smartpqi: Fix disable_managed_interrupts

Correct blk-mq registration issue with module parameter
disable_managed_interrupts enabled.

When we turn off the default PCI_IRQ_AFFINITY flag, the driver needs to
register with blk-mq using blk_mq_map_queues(). The driver is currently
calling blk_mq_pci_map_queues() which results in a stack trace and possibly
undefined behavior.

scsi: smartpqi: Fix logical volume rescan race condition

Correct rescan flag race condition.

Multiple conditions are being evaluated before notifying OS to do a rescan.

Driver will skip rescanning the device if any one of the following
conditions are met:

- Devices that have not yet been added to the OS or devices that have been
removed.

- Devices which are already marked for removal or in the phase of removal.

Under very rare conditions, after logical volume size expansion, the OS
still sees the size of the logical volume which was before expansion.

The rescan flag in the driver is used to signal the need for a logical
volume rescan. A race condition can occur in the driver, and it leads to
one thread overwriting the flag inadvertently. As a result, driver is not
notifying the OS SML to rescan the logical volume.

Move device->rescan update into new function pqi_mark_volumes_for_rescan()
and protect with a spin lock.

Move check for device->rescan into new function pqi_volume_rescan_needed()
and protect function call with a spin_lock.

scsi: core: Improve type safety of scsi_rescan_device()

Most callers of scsi_rescan_device() have the scsi_device pointer readily
available. Pass a struct scsi_device pointer to scsi_rescan_device()
instead of a struct device pointer. This change prevents that a pointer to
another struct device would be passed accidentally to scsi_rescan_device().

Remove the scsi_rescan_device() declaration from the scsi_priv.h header
file since it duplicates the declaration in <scsi/scsi_host.h>.

scsi: smartpqi: Replace one-element arrays with flexible-array members

One-element arrays are deprecated, and we are replacing them with flexible
array members instead. So, replace one-element arrays with flexible-array
members in a couple of structures, and refactor the rest of the code,
accordingly.

This helps with the ongoing efforts to tighten the FORTIFY_SOURCE routines
on memcpy().

This results in no differences in binary output.

scsi: smartpqi: Replace all non-returning strlcpy() with strscpy()

strlcpy() reads the entire source buffer first. This read may exceed the
destination size limit. This is both inefficient and can lead to linear
read overflows if a source string is not NUL-terminated [1]. In an effort
to remove strlcpy() completely [2], replace strlcpy() here with strscpy().
No return values were used, so direct replacement is safe.

scsi: smartpqi: Stop sending driver-initiated TURs

Stop sending driver-initiated TURs to physical devices during driver
load/rescan.

Note: This does not affect SML initiated TURs.

Some Linux kernels can cause lengthy delays in OS boot if the kernel
detects that a drive is being sanitized/erased. We were using TURs to
detect if a sanitize/erase was in progress.

Some devices do not return the TUR in a timely manner, causing driver
load/rescan stalls.

Correct OOPs on ARM servers during driver init.

The driver attempts to update FW with max_feature_supported value using a
writew() kernel call using a byte aligned address. This fails on some ARM
systems.

Reduce CPU contention when incrementing variable raid_bypass_cnt.

Remove the atomic operations for this variable by changing the atomic to an
unsigned int and replace atomic operations with standard operations. The
value is only checked that it is increasing and accuracy is not required.

Map full length of PCI BAR 0 at driver init.

During driver initialization, the driver must make a kernel call to map the
controller registers into kernel address space. A parameter to this call
is the length of the memory to be mapped. The driver was specifying the
wrong length.

overflow: Add struct_size_t() helper

While struct_size() is normally used in situations where the structure
type already has a pointer instance, there are places where no variable
is available. In the past, this has been worked around by using a typed
NULL first argument, but this is a bit ugly. Add a helper to do this,
and replace the handful of instances of the code pattern with it.

One-element arrays are deprecated, and we are replacing them with flexible
array members instead. So, replace one-element array with flexible-array
member in struct report_log_lun_list.

This helps with the ongoing efforts to tighten the FORTIFY_SOURCE routines
on memcpy().

scsi: smartpqi: Add ctrl ready timeout module parameter

Allow user to override the default driver timeout for controller ready.

There are some rare configurations which require the driver to wait longer
than the normal 3 minutes for the controller to complete its bootup
sequence and be ready to accept commands from the driver.

The module parameter is:

ctrl_ready_timeout= { 0 | 30-1800 }

and specifies the timeout in seconds for the driver to wait for controller
ready. The valid range is 0 or 30-1800. The default value is 0, which
causes the driver to use a timeout of 180 seconds (3 minutes).

scsi: smartpqi: Add module param to disable managed ints

Allow SMP affinity to be changeable by disabling managed interrupts.

On distributions where the driver is enabled for multi-queue support the
driver utilizes kernel managed interrupts, which automatically distributes
interrupts to all available CPUs and assigns SMP affinity.

On most distributions, the affinity can not be changed by the user.

This change will allow managed interrupts to be disabled by the user via a
module parameter while still allowing multi-queue support to function
properly.

Use the module parameter disable_managed_interrupts=1

Correct a rare stale RAID map access when performing AIO during a RAID
configuration change.

A race condition in the driver could cause it to access a stale RAID map
when a logical volume is reconfigured.

Modify the driver logic to invalidate a RAID map very early when a RAID
configuration change is detected and only switch to a new RAID map after
the driver detects that the RAID map has changed.

scsi: smartpqi: Fix DMA direction for RAID requests

Correct a SOP READ and WRITE DMA flags for some requests.

This update corrects DMA direction issues with SCSI commands removed from
the controller's internal lookup table.

Currently, SCSI READ BLOCK LIMITS (0x5) was removed from the controller
lookup table and exposed a DMA direction flag issue.

SCSI READ BLOCK LIMITS was recently removed from our controller lookup
table so the controller uses the respective IU flag field to set the DMA
data direction. Since the DMA direction is incorrect the FW never completes
the request causing a hang.

Some SCSI commands which use SCSI READ BLOCK LIMITS

* sg_map
* mt -f /dev/stX status

After updating controller firmware, users may notice their tape units
failing. This patch resolves the issue.

Also, the AIO path DMA direction is correct.

The DMA direction flag is a day-one bug with no reported BZ.

scsi: smartpqi: Fix PCI control linkdown system hang

Fail all outstanding requests after a PCI linkdown.

Block access to device SCSI attributes during the following conditions:

"Cable pull" is called PQI_CTRL_SURPRISE_REMOVAL.

"PCIe Link Down" is called PQI_CTRL_GRACEFUL_REMOVAL.

Block access to device SCSI attributes during and in rare instances when
the controller goes offline.

Either outstanding requests or the access of SCSI attributes post linkdown
can lead to a hang.

Post linkdown, driver does not fail the outstanding requests leading to
long wait time before all the IOs eventually fail.

Also access of the SCSI attributes by host applications can lead to a
system hang.

Add driver support for up to 256 LUNs per device.

Update AIO path to pass the appropriate LUN number for base-code to target
the correct LUN.

Update RAID IO path to pass the appropriate LUN number for FW to target the
correct LUN.

Pass the correct LUN number while doing a LUN reset.

Count the outstanding commands based on LUN number. While removing a
Multi-LUN device, wait for all outstanding commands to complete for all
LUNs.

Add Feature bit support.

scsi: smartpqi: Shorten drive visibility after removal

Check the response code returned from the LUN reset task management
function and if it indicates the LUN is not valid, do not retry.

Reduce rescan worker delay to 5 seconds for the event handler only.

The removal of a drive from the OS could have been delayed up to 30 seconds
after being physically pulled.

The driver was retrying a LUN reset 3 times even though the return code
indiciated the LUN was no longer valid. There was a 10 second delay between
each retry. Additionally, the rescan worker was scheduled to run 10 seconds
after the driver received the event.

scsi: smartpqi: Fix unused variable pqi_pm_ops for clang

Driver added a new dev_pm_ops structure used only if CONFIG_PM is set. The
CONFIG_PM MACRO needed to be moved up in the code to avoid the compiler
warnings. The hunk to move the location was missing from the above patch.

Found by kernel test robot by building driver with CONFIG_PM disabled.

Correct lsscsi -t output for newer controllers that support 16-byte WWID in
the SAS address field. lsscsi -t was displaying all zeros for SAS
addresses.

When we added support to smartpqi for 16-byte WWIDs in the RPL data for
newer controllers, we were copying the wrong part of the 16-byte WWID to
the SAS address field.

scsi: smartpqi: Fix NUMA node not updated during init

Correct NUMA node association when calling pqi_pci_probe().

In the function pqi_pci_probe(), the driver makes an OS call to get the
NUMA node associated with a controller. If the call returns that there is
no associated node, the driver attempts to set it to node 0. The problem is
that a different local variable (cp_node) was being used to do this, but
the original local variable (node) was still being used in the call to
pqi_alloc_ctrl_info().

The value of "node" is not updated if the conditional after the call to
dev_to_node() evaluates to TRUE.

scsi: smartpqi: Expose SAS address for SATA drives

Remove UNIQUE_WWID_IN_REPORT_PHYS_LUN PQI feature.

This feature was originally added to solve a problem with NVMe drives, but
this problem has since been solved a different way, so this PQI feature is
no longer required for any type of drive.

The kernel was not creating symbolic links in sysfs between SATA drives and
their enclosure.

The driver was enabling the UNIQUE_WWID_IN_REPORT_PHYS_LUN PQI feature,
which causes the FW to return a WWID for SATA drives that is derived from a
unique ID read from the SATA drive itself. The driver was exposing this
WWID as the drive's SAS address. However, because this SAS address does not
match the SAS address returned by an enclosure's SES Page 0xA data, the
Linux kernel was not able to match a SATA drive with its enclosure.

scsi: smartpqi: Speed up RAID 10 sequential reads

Use all data disks for sequential read operations.

Testing discovered inconsistent performance on RAID 10 volumes when
performing 256K sequential reads. The driver was only using a single
tracker to determine which physical drive to send a request to for AIO
requests.

Change the single tracker (next_bypass_group) to an array of trackers based
on the number of data disks in a row of the RAID map.

scsi: smartpqi: Fix kdump issue when controller is locked up

Avoid dropping into shell if the controller is in locked up state.

Driver issues SIS soft reset to bring back the controller to SIS mode while
OS boots into kdump mode.

If the controller is in lockup state, SIS soft reset does not work.

Since the controller lockup code has not been cleared, driver considers the
firmware is no longer up and running. Driver returns back an error code to
OS and the kdump fails.

scsi: smartpqi: Update volume size after expansion

After modifying logical volume size, lsblk command still shows previous
size of logical volume.

When the driver gets any event from firmware it schedules rescan worker
with delay of 10 seconds. If array expansion is so quick that it gets
completed in a second, the driver could not catch logical volume expansion
due to worker delay.

Since driver does not detect volume expansion, driver would not call
rescan device to update new size to the OS.

into the suspend (S3) state, the BMIC flush cache command will indicate a

scsi: smartpqi: Resolve delay issue with PQI_HZ value

Change PQI_HZ to HZ.

PQI_HZ macro was set to 1000 when HZ value is less than 1000. By default,
PQI_HZ will result into a delay of 10 seconds(for kernel, which has HZ =
100). So in this case when firmware raises an event, rescan worker will be
scheduled after a delay of (10 x PQI_HZ) = 100 seconds instead of 10
seconds.

Also driver uses PQI_HZ at many instances, which might result in some other
issues with respect to delay.

Return DID_NO_CONNECT when a path failure is detected.

When a path fails during I/O and AIO path gets disabled for a multipath
device, the I/O was retried in the RAID path slowing down path fail
detection. Returning DID_NO_CONNECT allows multipath to switch paths more
quickly.

scsi: smartpqi: Fix duplicate device nodes for tape changers

Stop the OS from re-discovering multiple LUNs for tape drive and medium
changer.

Duplicate device nodes for Ultrium tape drive and medium changer are being
created.

The Ultrium tape drive is a multi-LUN SCSI target. It presents a LUN for
the tape drive and a 2nd LUN for the medium changer. Our controller FW
lists both LUNs in the RPL results.

As a result, the smartpqi driver exposes both devices to the OS. Then the
OS does its normal device discovery via the SCSI REPORT LUNS command, which
causes it to re-discover both devices a 2nd time, which results in the
duplicate device nodes.

scsi: smartpqi: Avoid failing I/Os for offline devices

Prevent kernel crash by failing outstanding I/O request when the OS takes
device offline.

When posted I/Os to the controller's inbound queue are not picked by the
controller, the driver will halt the controller and take the controller
offline.

When the driver takes the controller offline, the driver will fail all the
outstanding requests which can sometimes lead to an OS crash.

scsi: smartpqi: Add TEST UNIT READY check for SANITIZE operation

Send a TEST UNIT READY to HBA disks and do not present them to the OS if
0x02/0x04/0x1b (SANITIZE IN PROGRESS) is returned.

During boot-up, some OSes appear to hang when there are one or more disks
undergoing a sanitize operation.

According to SCSI SBC4 specification section 4.11.2 "Commands allowed
during SANITIZE", some SCSI commands are permitted, but read/write
operations are not.

When the OS attempts to read the disk partition table a CHECK CONDITION ASC
0x04 ASCQ 0x1b is returned which causes the OS to retry the read until
SANITIZE has completed. This can take hours.

According to document HPE Smart Storage Administrator User Guide, during
the sanitize erase operation, the drive is unusable. I.e. the expected
behavior for SANITIZE is the that disk remains offline even after SANITIZE
has completed. The customer is expected to re-enable the disk using the
management utility.

scsi: smartpqi: Update LUN reset handler

Enhance check for commands queued to the controller. Add new function
pqi_nonempty_inbound_queue_count() that will wait for all I/O queued for
submission to controller across all queue groups to drain. Add helper
functions to obtain queue command counts for each queue group. These
queues should drain quickly as they are already staged to be submitted down
to the controller's IB queue.

Enhance check for outstanding command completion. Update the count of
outstanding commands while waiting. This value was not re-obtained and was
potentially causing infinite wait for all completions.

scsi: smartpqi: Add controller handshake during kdump

Correct kdump hangs when controller is locked up.

There are occasions when a controller reboot (controller soft reset) is
issued when a controller firmware crash dump is in progress.

This leads to incomplete controller firmware crash dump:

- When the controller crash dump is in progress, and a kdump is initiated,
the driver issues inbound doorbell reset to bring back the controller in
SIS mode.

- If the controller is in locked up state, the inbound doorbell reset does
not work causing controller initialization failures. This results in the
driver hanging waiting for SIS mode.

To avoid an incomplete controller crash dump, add in a controller crash
dump handshake:

- Controller will indicate start and end of the controller crash dump by
setting some register bits.

- Driver will look these bits when a kdump is initiated. If a controller
crash dump is in progress, the driver will wait for the controller crash
dump to complete before issuing the controller soft reset then complete
driver initialization.

scsi: smartpqi: Replace one-element array with flexible-array member

There is a regular need in the kernel to provide a way to declare having a
dynamically sized set of trailing elements in a structure. Kernel code
should always use "flexible array members"[1] for these cases. The older
style of one-element or zero-length arrays should no longer be used[2].

Refactor the code a bit according to the use of a flexible-array member in
struct pqi_event_config instead of a one-element array, and use the
struct_size() helper.

This helps with the ongoing efforts to globally enable -Warray-bounds and
get us closer to being able to tighten the FORTIFY_SOURCE routines on
memcpy().

This issue was found with the help of Coccinelle and audited and fixed,
manually.

scsi: smartpqi: Fix blocks_per_row static checker issue

Dan Carpenter found a possible divide by 0 issue in the smartpqi driver in
functions pci_get_aio_common_raid_map_values() and pqi_calc_aio_r5_or_r6().
The variable rmd->blocks_per_row is used as a divisor and could be 0.

Using rmd->blocks_per_row as a divisor without checking
it for 0 first.

Correct these possible divide by 0 conditions by insuring that
rmd->blocks_per_row is not zero before usage. The check for non-0 was too
late to prevent a divide by 0 condition. Add in a comment to explain why
the check for non-zero is necessary. If the member is 0, return
PQI_RAID_BYPASS_INELIGIBLE before any division is performed.

scsi: smartpqi: Update enclosure identifier in sysfs

Update enclosure identifier field corresponding to physical devices in
lsscsi/sysfs.

During device add the SCSI devtype is filled in during slave_configure().
However, when pqi_scsi_update_device() runs (REGNEWD) the firmware returns
zero for the SCSI devtype field, and valid devtype is overwritten by
zero. Due to this, lsscsi output shows wrong values.

LUN resets can take longer to complete. Adding in more driver logging helps
show where the driver is in the reset process.

Add a timeout in pqi_device_wait_for_pending_io() to cap how long the
driver will wait for outstanding commands.

scsi: smartpqi: Fix driver synchronization issues

- Synchronize OFA and controller offline events. Prevent I/O during the
above conditions.

- Cleanup pqi_device_wait_for_pending_io() by checking the
device->scsi_cmds_outstanding instead of walking the device's list of
commands.

- Stop failing all I/O for all devices. This was causing OS to retry them,
delaying OFA.

- Clean up cache flush. The controller is checked for offline status in
lower level functions.

Change return type from EINPROGRESS to EBUSY to signal applications to
retry a REGNEWD if the driver cannot process the REGNEWD. Events such as
OFA, suspend, and shutdown return EINPROGRESS if a scan is currently
running. This prevents applications from immediately retrying REGNEWD.

Schedule a new REGNEWD if system low on memory.

scsi: smartpqi: Update soft reset management for OFA

Cleanup soft reset code for Online Firmware Activation (OFA). OFA allows
controller firmware updates without a reboot.

OFA updates require an on-line controller reset to activate the updated
firmware. There were some missing actions for some of the reset cases. The
controller is first set back to sis mode before returning to pqi mode.
Check to ensure the controller is in sis mode.

Release QRM memory (OFA buffer) on OFA error conditions. Clean up
controller state which can cause a kernel panic upon reboot after an
unsuccessful OFA.

scsi: smartpqi: Add stream detection

Enhance performance by adding sequential stream detection for RAID5/RAID6
sequential write requests. Reduce stripe lock contention with full-stripe
write operations.

There is one common stripe lock for each RAID volume that can be set by
either the RAID engine or the AIO engine. The AIO path has I/O request
sizes well below the stripe size resulting in many Read-Modify-Write
operations.

Sending the request to the RAID engine allows for coalescing requests into
full stripe operations resulting in reduced Read-Modify-Write operations.

While failing queued I/Os in TMF path, there was a request leak and hence
stale entries in request pool with ref count being non-zero. In shutdown
path we have a BUG_ON to catch stuck I/O either in firmware or in the
driver. The stale requests caused a system crash. The I/O request pool
leakage also lead to a significant performance drop.

Correct rmmod hangs when using HBA disks with write cache enabled.

Do not set controller flag "in_shutdown" during rmmod. SCSI SYNCHRONIZE
CACHE(10) and SCSI SYNCHRONIZE CACHE(16) requests were blocked with
SCSI_MLQUEUE_HOST_BUSY.

PCI: Convert PCIe capability PCIBIOS errors to errno

The PCI config accessors (pci_read_config_word(), et al) return
PCIBIOS_SUCCESSFUL (zero) or positive error values like
PCIBIOS_FUNC_NOT_SUPPORTED.

The PCIe capability accessors (pcie_capability_read_word(), et al)
similarly return PCIBIOS errors, but some callers assume they return
generic errno values like -EINVAL.

For example, the Myri-10G probe function returns a positive PCIBIOS error
if the pcie_capability_clear_and_set_word() in pcie_set_readrq() fails:

A positive return from a PCI driver probe function would cause a "Driver
probe function unexpectedly returned" warning from local_pci_probe()
instead of the desired probe failure.

Convert PCIBIOS errors to generic errno for all callers of:

pcie_capability_read_word
pcie_capability_read_dword
pcie_capability_write_word
pcie_capability_write_dword
pcie_capability_set_word
pcie_capability_set_dword
pcie_capability_clear_word
pcie_capability_clear_dword
pcie_capability_clear_and_set_word
pcie_capability_clear_and_set_dword

that check the return code for anything other than zero.

Basically, the fact that the coherent DMA mask value wasn't set caused the
driver to fall back to SWIOTLB when SME is active.

For correct operation, lets call the dma_set_mask_and_coherent() to
properly set the mask for both streaming and coherent, in order to inform
the kernel about the devices DMA addressing capabilities.

scsi: smartpqi: Reporting unhandled SCSI errors

When a HARDWARE_ERROR is triggered for ASC=0x3e, the existing code is only
considering the case where ASCQ=0x1.

According to the http://www.t10.org/lists/asc-num.htm#ASC_3E specification,
other values may occur like a timeout (ASCQ=0x2).

This patch prints an error message when a non-handled message is received.
This can help diagnose a possible misbehavior of the controller or a
missing implementation in the Linux kernel.

This patch keeps the exact same error handling but prints a message if an
ASCQ != 1 is reported.

scsi: smartpqi: Reporting 'logical unit failure'

When the HARDWARE_ERROR/0x3e/0x1 case is triggered, the logical volume
is offlined. When reading the kernel log, the reason why the device
got offlined isn't reported to the user. This situation makes it
difficult for admins to root cause.

Log a message when this condition occurs.

The root cause is that the _IOC macro can generate really large numbers,
which don't fit into type 'int', which is used for the cmd parameter in
the ioctls in scsi_host_template. My research into how GCC and Clang are
handling this at a low level didn't prove fruitful. However, looking at
the rest of the kernel tree, all ioctls use an 'unsigned int' for the
cmd parameter, which will fit all of the _IOC values in the scsi/ata
subsystems.

Make that change because none of the ioctls expect a negative value for
any command, it brings the ioctls inline with the reset of the kernel,
and it removes ambiguity, which is never good when dealing with compilers.

- when OFA event occurs, driver will stop traffic to RAID/HBA path. Driver
waits for all the outstanding requests to complete.
- Driver sends OFA event acknowledgment to firmware.
- Driver will wait until the new firmware is up and running.

- Driver calls SIS/PQI initialization and rescans the device list.

scsi: smartpqi: correct lun reset issues

Problem:
The Linux kernel takes a logical volume offline after a LUN reset. This is
generally accompanied by this message in the dmesg output:

Device offlined - not ready after error recovery

Root Cause:
The root cause is a "quirk" in the timeout handling in the Linux SCSI
layer. The Linux kernel places a 30-second timeout on most media access
commands (reads and writes) that it send to device drivers. When a media
access command times out, the Linux kernel goes into error recovery mode
for the LUN that was the target of the command that timed out. Every
command that timed out is kept on a list inside of the Linux kernel to be
retried later. The kernel attempts to recover the command(s) that timed out
by issuing a LUN reset followed by a TEST UNIT READY. If the LUN reset and
TEST UNIT READY commands are successful, the kernel retries the command(s)
that timed out.

Each SCSI command issued by the kernel has a result field associated with
it. This field indicates the final result of the command (success or
error). When a command times out, the kernel places a value in this result
field indicating that the command timed out.

The "quirk" is that after the LUN reset and TEST UNIT READY commands are
completed, the kernel checks each command on the timed-out command list
before retrying it. If the result field is still "timed out", the kernel
treats that command as not having been successfully recovered for a
retry. If the number of commands that are in this state are greater than
two, the kernel takes the LUN offline.

Fix:
When our RAIDStack receives a LUN reset, it simply waits until all
outstanding commands complete. Generally, all of these outstanding commands
complete successfully. Therefore, the fix in the smartpqi driver is to
always set the command result field to indicate success when a request
completes successfully. This normally isn’t necessary because the result
field is always initialized to success when the command is submitted to the
driver. So when the command completes successfully, the result field is
left untouched. But in this case, the kernel changes the result field
behind the driver’s back and then expects the field to be changed by the
driver as the commands that timed-out complete.

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the

scsi: smartpqi: fully convert to the generic DMA API

The driver is currently using an odd mix of legacy PCI DMA API and
generic DMA API calls, switch it over to the generic API entirely.

scsi: smartpqi: fix critical ARM issue reading PQI index registers

Use the readl() kernel function to read all index registers. For ARM
systems, this function includes a read memory barrier that eliminates ci/pi
corruption.

The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

kzalloc(a * b, gfp)

with:
kcalloc(a * b, gfp)

as well as handling cases of:

kzalloc(a * b * c, gfp)

with:

kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

blk-mq: Allow PCI vector offset for mapping queues

The PCI interrupt vectors intended to be associated with a queue may
not start at 0; a driver may allocate pre_vectors for special use. This
patch adds an offset parameter so blk-mq may find the intended affinity
mask and updates all drivers using this API accordingly.

scsi: smartpqi: workaround fw bug for oq deletion

Skip deleting PQI operational queues when there is an error creating a
new queue group. It's not really necessary to delete the queues anyway
because they get deleted during the PQI reset that is part of the error
recovery path.

scsi: smartpqi: update kexec and power down support

Add PQI reset to driver shutdown callback to work around controller bug.

During an 1.) OS shutdown or 2.) kexec outside of a kdump, the Linux
kernel will clear BME on our controller.

If BME is cleared during a controller/host PCIe transfer, the controller
will lock up.

So we perform a PQI reset in the driver's shutdown callback function to
eliminate the possibility of a controller/host PCIe transfer being
active when the kernel clears BME immediately after calling the driver's
shutdown callback.

unused code silently.
