wifi: ath9k: delete some unused/duplicate macros

The rate macros are for AR9002 and not correct for AR9003.

The AGC 3 macros are unused and have incorrect values, at least for
QCN5502, where AR_AGC3_BASE should be 0x2de00.

This change does not appear to affect the final binary.

ath9k: fix QCA9561 PA bias level

This patch fixes an invalid TX PA DC bias level on QCA9561, which
results in a very low output power and very low throughput as devices
are further away from the AP (compared to other 2.4GHz APs).

This patch was suggested by Felix Fietkau, who noted[1]:
"The value written to that register is wrong, because while the mask
definition AR_CH0_TOP2_XPABIASLVL uses a different value for 9561, the
shift definition AR_CH0_TOP2_XPABIASLVL_S is hardcoded to 12, which is
wrong for 9561."

In real life testing, without this patch the 2.4GHz throughput on
Yuncore XD3200 is around 10Mbps sitting next to the AP, and closer to
practical maximum with the patch applied.

ath9k_hw: fix duplicate (and partially wrong) definition of AR_CH0_THERM

AR_PHY_65NM_CH0_THERM and AR_CH0_THERM were supposed to refer to the
same register, however they had different SREV checks.

Remove the duplicate and use the checks. Since there were other SREV
checks present in the only place that uses this, this will probaby not
affect runtime behavior.

ath9k: Identify baseband watchdog signatures

A full HW reset is not required for all baseband watchdog
signatures. Some BB watchdog updates are benign and can
be discarded, some require re-programming of certain registers
and others require a chip reset.

This patch adds a routine to identify such signatures.

ath9k: Fix issue with MCS15

On some boards which are based on AR9300, AR9580 or
AR9550, MCS15 usage is problematic.

This is because these boards use a "frequency doubler",
which doubles the refclk to get better EVM, but causes
spurs. Handle this properly in the driver to recover
throughput.

When antenna diversity combining is enabled in the EEPROM,
the initial values for the MAIN/ALT config have to be
programmed correctly. This patch adds it for AR9285.

Since the diversity combining macros are common to all chip
families, remove the redundant AR9285 macros and move the
definitions to phy.h.

ath9k_hw: Fix wrong peak detector DC offset

An issue is reported in AR9462 & AR9565 that NF_cal_not_done is
not observed when HW peak detector calibration is disabled. At that
state, the HW is stuck at NF calibration which prevents tx output.
The root cause is wrong peak detector offset calibrated by HW. To
resolve this issue, peak detector calibration is done manually by SW
for AR9462 and AR9565.

ath9k_hw: Fix regression of register offset for AR9003 chips

My recent commits (3782c69d, 324c74a) introduced regression
for register offset selection that based on the macversion.
Not using parentheses in proper manner for ternary operator
leads to select wrong offset for the registers.

This issue was observed with AR9462 chip that immediate disconnect
after the association with the following message

ieee80211 phy3: wlan0: Failed to send nullfunc to AP 00:23:69:12:ea:47
after 500ms, disconnecting.

ath9k_hw: Cleanup Tx calibrations for AR9003 chips

Currently Tx IQ calibration is enabled by default for all AR9003
chips. But for AR9480, the calibration status should be read from
chip after processing ini. And also the carrier leak calibration
status is checked during init cal. As the init_cal is being called
for fast channel change too, the tx_cl status only be read after
full reset. Hence moving that into process ini function.

ath9k_hw: Improve fast channel change for AR9003 chips

In order to reduce the overall scan time, fast channel change
should be implemented properly. This patch adds fast channel
change support across band switch or channel mode switch
instead of doing full chip reset. During the fastcc, tx iqcal
measurements are preserved and will be reloaded after successful
the channel change.

This patch also addressed fast channel issue where the STA can not
see APs in higher than operating channel on 5GHz band after
the association.

ath9k_hw: Fix Tx IQ Calibration hang issue in AR9003 chips

On AR9003 chips, doing three IQ calibrations will possibly cause chip
in stuck state. In noisy environment, chip could receive
a packet during the middle of three calibrations and it causes
the conflict of HW access and the eventual failure. It also
causes IQ calibration outliers which results in poor Tx EVM.

The IQ Cal procedure is after resetting the chip, run IQ cal 3 times
per each cal cycle and find the two closest readings and average of two.
The advantage of running Tx IQ cal more than once is that we can compare
calibration results for the same gain setting over multiple iterations.
Most of the cases the IQ failures were observed after first pass.

For the AR9485 and later chips, Tx IQ Calibration is performed along
with AGC cal. But for pre-AR9485 chips, Tx IQ cal HW has to be separated
from the rest of calibration HW to avoid chip hang. After all
calibrations are done in HW, we can start SW post-processing.
By doing this way, we minimize the SW difference among all chips.

The order of calibration (run IQ cal before other calibration) is also
needed to avoid chip hang for chips before AR9485. This issue was
originally observed with AR9382.

During the issue kernel log was filled with following message

ath9k_hw: fix PA predistortion training power selection

The EEPROM contains scale factors for the tx power, which define
the range of allowable difference between target power and training
power. If the difference is too big, PA predistortion cannot be used.
For 2.4 GHz there is only one scale factor, for 5 GHz there are
three, depending on the specific frequency range.
