MIPS: lantiq: Remove unused function pointer variables

Ever since introduction in the commit e47d488935ed ("MIPS: Lantiq: Add
PCI controller support.") ltqpci_plat_dev_init has been unused. In
57c8cb8f2429 ("MIPS: pci: convert lantiq driver to OF") also
ltq_pci_plat_arch_init was introduced.

With those commit being more than 10 years ago, it seem neither is
going to get used anytime soon. Thus, remove both unused function
pointer variables.

MIPS: lantiq: Remove unnecessary include of <linux/of_irq.h>

arch/mips/pci/fixup-lantiq.c requires nothing from <linux/of_irq.h>, so
remove the include of it.

MIPS: lantiq: Fix pcibios_plat_dev_init() "no previous prototype" warning

After bbd8810d3998 ("PCI: Remove unused includes and superfluous struct
declaration"), <linux/of_pci.h> no longer includes <linux/pci.h>, which
provides the extern declarations for pcibios_plat_dev_init() and
pcibios_map_irq() via <asm/pci.h>.

This results in these new warnings:

arch/mips/pci/fixup-lantiq.c:13:5: warning: no previous prototype for 'pcibios_plat_dev_init' [-Wmissing-prototypes]
arch/mips/pci/fixup-lantiq.c:24:5: warning: no previous prototype for 'pcibios_map_irq' [-Wmissing-prototypes]

Include <linux/pci.h> directly to get these declarations.

MIPS: PCI: fix pcibios_map_irq section mismatch

Drop the __init from pcibios_map_irq() to make this section mis-
match go away:

The function pcibios_scanbus() references
the function __init pcibios_map_irq().
This is often because pcibios_scanbus lacks a __init

MIPS: PCI: Move map_irq() hooks out of initdata

04c81c7293df ("MIPS: PCI: Replace pci_fixup_irqs() call with host bridge
IRQ mapping hooks") moved the PCI IRQ fixup to the new host bridge
map/swizzle_irq() hooks mechanism. Those hooks can also be called after
boot, when all the __init/__initdata/__initconst sections have been freed.
Therefore, functions called by them (and the data they refer to) must not
be marked as __init/__initdata/__initconst lest compilation trigger section
mismatch warnings.

Fix all the board files map_irq() hooks by simply removing the respective
__init/__initdata/__initconst section markers and by adding another
persistent hook IRQ map for the txx9 board files.

of/irq: Create of_irq_parse_and_map_pci() to consolidate arch code.

Several architectures open code effectively the same code block for
finding and mapping PCI irqs. This patch consolidates it down to a
single function.

All the callers of irq_create_of_mapping() pass the contents of a struct
of_phandle_args structure to the function. Since all the callers already
have an of_phandle_args pointer, why not pass it directly to
irq_create_of_mapping()?

struct of_irq and struct of_phandle_args are exactly the same structure.
This patch makes the kernel use of_phandle_args everywhere. This in
itself isn't a big deal, but it makes some follow-on patches simpler.

of/irq: Rename of_irq_map_* functions to of_irq_parse_*

The OF irq handling code has been overloading the term 'map' to refer to
both parsing the data in the device tree and mapping it to the internal
linux irq system. This is probably because the device tree does have the
concept of an 'interrupt-map' function for translating interrupt
references from one node to another, but 'map' is still confusing when
the primary purpose of some of the functions are to parse the DT data.

This patch renames all the of_irq_map_* functions to of_irq_parse_*
which makes it clear that there is a difference between the parsing
phase and the mapping phase. Kernel code can make use of just the
parsing or just the mapping support as needed by the subsystem.

The patch was generated mechanically with a handful of sed commands.

Implement support for OF inside the lantiq PCI driver. The patch also splits
pcibios_plat_dev_init and pcibios_map_irq out into their own file to accomodate
coexistance with the upcoming pcie driver.
