sys: Remove $FreeBSD$: two-line .h pattern

Remove /^\s*\*\n \*\s+\$FreeBSD\$$\n/

ddb: print the actual syscall name

Some architectures will pretty-print a system call trap in the
backtrace. Rather than printing the symbol, use the syscallname()
function to pull the string from the sv_syscallnames array corresponding
to the process. This simplifies the function somewhat.

Mostly, this will result in dropping the "sys" prefix, e.g. "sys_exit"
will now be printed simply as "exit".

Make two minor tweaks to the function signature: use a u_int for the
syscall number since this is a more correct type (see the 'code' member
of struct syscall_args), and make the thread pointer the first argument.
The latter is more natural and conventional.

ddb: de-duplicate decode_syscall()

Only i386 and amd64 print the decoded syscall name in the backtrace.
This de-duplication facilitates further changes and adoption by other
platforms.

ddb: finish converting boolean values.

The replacement started at r283088 was necessarily incomplete without
replacing boolean_t with bool. This also involved cleaning some type
mismatches and ansifying old C function declarations.

Fixed some style bugs in the removal of __P(()). Continuation lines
were not outdented to preserve non-KNF lining up of code with parentheses.
Switch to KNF formatting.

Remove __P.

$Id$ -> $FreeBSD$

Fixed -Wcast-qual warnings.

Fix warnings in preparation for adding -Wall -Wcast-qual to the
kernel compile.

This commit includes significant work to proper handle const arguments
for the DDB symbol routines.

Changes to support -Wall, -Wcast-qual. Had to make physical code changes
in db_lookup() to avoid the *cp = 0 / *cp = ':' combo ( temporary
nul-terminate-then-restore-original ) on a const char * string.

Supply the symbol table as an argument to X_db_symbol_values (required for elf).

Some staticized variables were still declared to be extern.

Don't depend on gcc's feature of interpreting `int foo(c) char c; ...'
as `int foo(char c) ...' if there is a bogus prototype `int foo(char c);'
in scope.

Back out part 1 of the MCFH that changed $Id$ to $FreeBSD$. We are not
ready for it yet.

Make the long-awaited change from $Id$ to $FreeBSD$

This will make a number of things easier in the future, as well as (finally!)
avoiding the Id-smashing problem which has plagued developers for so long.

Boy, I'm glad we're not using sup anymore. This update would have been
insane otherwise.

Staticized and '#ifdef notused' stuff we don't use.

Completed function declarations and/or added prototypes.
Removed `extern' from prototypes.
Sorted prototypes.
Uniformized idempotency ifdefs.

Remove trailing whitespace.

Add and move declarations to fix all of the warnings from `gcc -Wimplicit'
(except in netccitt, netiso and netns) and most of the warnings from
`gcc -Wnested-externs'. Fix all the bugs found. There were no serious
ones.

Makes gcc silent in sys/ddb.

1) cleaned up after Garrett - fixed more redundant declarations, changed
use of timeout_t -> timeout_func_t in aha1542 and aha1742 drivers.
2) fix a bug in the portalfs that was uncovered by better prototyping -
specifically, the time must be converted from timeval to timespec
before storing in va_atime.
3) fixed/added some miscellaneous prototypes

Fix up some sloppy coding practices:

- Delete redundant declarations.
- Add -Wredundant-declarations to Makefile.i386 so they don't come back.
- Delete sloppy COMMON-style declarations of uninitialized data in
header files.
- Add a few prototypes.
- Clean up warnings resulting from the above.

NB: ioconf.c will still generate a redundant-declaration warning, which
is unavoidable unless somebody volunteers to make `config' smarter.

Make all headers idempotent. All incorrect common data moved out of header
files and into a related source file.

Removed all patch kit headers, sccsid and rcsid strings, put $Id$ in, some
minor cleanup. Added $Id$ to files that did not have any version info, etc
