mdoc: drop redundant .Pp and .LP calls

They have no effect when coming in pairs, or before .Bl/.Bd

Fix typos, spelling, formatting and mdoc mistakes found by Nobuyuki while
translating these manual pages. Minor corrections by me.

mdoc(7) police: s/BSD/.Bx/ where appropriate.

Removed whitespace at end-of-line; no content changes. I simply did
cd src/share; find man[1-9] -type f|xargs perl -pi -e 's/[ \t]+$//'

BTW, what editors are the culprits? I'm using vim and it shows
me whitespace at EOL in troff files with a thick blue block...

mdoc(7) police: removed HISTORY info from the .Os call.

Remove the MD part from the document title.

mdoc(7) police: Change -filled displays (which just happen
to be the same as -ragged in the current implementation) to
-ragged. With mdocNG, -filled displays produce the correct
output, formatted and justified to both margins.

mdoc(7) police: do not split author names in the AUTHORS section.

Remove stray "cam" in the example device lines.

Discourage usage of controller in kernel config files. Reflect this
by changing ``controller'' to ``device'' as per GENERIC and LINT.
